Payment Policy

  • For those individuals without insurance coverage (private pay), payment in full is expected at the time of service. There will be an additional charge for any other testing.
  • We have contracted with several insurance plans. These include, but are not limited to, the following companies:

AARP -- Acordia -- Acordia National/PEIA -- Aetna -- Blue Cross/Blue Shield -- Carelink -- Champva/Tricare -- Choice Care Network -- CIGNA -- 4Most -- Medicare -- Private HealthCare Systems -- Railroad Retirement Medicare -- SelectNET -- UMW The Funds -- WV Chips -- WV Children's Specialty Care -- WV Medicaid (children only)

Please check with your individual carrier to determine whether we are listed as a participating provider.

  • We ask that charges subject to deductibles and co-payments be paid at the time of service.
  • We must have complete billing information to submit a claim. Please bring insurance cards and photo ID for appointments. Be advised that your insurance may require the Social Security number for covered dependents.
  • We accept cash, check, debit cards and most major credit cards.
